- Enrollment is not complete until your ITA membership and payment are verified.

- Your paypal transaction reciept is your payment confirmation. Balvihar fee is non-refundable.

- If you get a "Grade or Class is full" message, please email to bvadmin@indiatemple.org to be placed on our waitlist. Currently Pre-K, 1st & 7th Grade are full. Based on teacher availablity and demand, we may add more sections to a grade. Waitlists are adjudicated on first come, first serve basis.

Yes, I am the parent or legal guardian of the above registered child (hereinafter "Participant").

I understand that Balvihar maybe conducted in an in person or distant learning format based on prevailing circumstances.

I understand that the Balvihar Contribution is not refundable under any circumstances.

Yes, I acknowledge that Balvihar and the India Temple Association (hereinafter the "ITA") are a religious, cultural and community service focused non-profit institution.

Yes, I give full permission for the child to participate in all activities conducted by the ITA. I will also ensure that the Participant attends the Balvihar classes, group activities or excursions regularly and ON TIME. I further agree to take personal interest in what the child is being taught in the class and to cooperate with the teachers/volunteers including READING weekly class and administrative emails.

Yes, I give the ITA and Balvihar program full permission to publish the name, photographs, class work, projects and videos of the Participants in any and all ITA literature, program books, websites, newsletters, and social media platforms such as but not limited to YouTube and Facebook. I also give ITA and Balvihar permission to conduct classes over an interactive distance learning platform and record these classes to be used for future viewing.

Yes, I understand that if I have provided the Participants email address above, I am giving the Balvihar and ITA permission to add my child to the Balvihar Programs email distribution lists and to send them emails.

Yes, I hereby accept and understand that the Balvihar Program is an educational program which operates in a local public school. I understand that contributing to a positive learning environment and behaving in a manner that adheres to the policies of the school district and policies of the Balvihar Program is required for all students and parents. Students and parents are expected to be respectful and courteous to the teachers, program volunteers, school staff and fellow students. Except in the case of an emergency and during lunch, students are prohibited from using cell phones at any time during the Balvihar session. Students and parents are expected to be mindful to not disturb or damage school property or act in a disruptive manner that could cause injury to themselves or others. Students and parents should not bring any items or substances to the school that would be deemed hazardous to children or would be deemed inappropriate in a school environment. Violation of these policies can result in harm to others, the program and/or potential loss of school use for the program. I understand that the Administrators of the Balvihar program reserve the right to expel any child or family from the program if they are found be in in violation of this code of conduct.

I understand that in order to be eligible to participate in the Annual Day Program my child must have a greater than 50% attendance rate in the program and commit to attend the number of practices sessions required by their performance coordinators.

Yes, I agree that in Consideration of the Participant being allowed to participate in the functions, programs and activities held by the ITA, the parent or legal guardian hereby agrees, on behalf of the Participant and his or her heirs, legal representative guests and invitees, that the Participant hereby accepts and assumes any and all risks resulting from attendance and participation at such functions and activities: the Participant hereby releases the ITA directors, officers, members, volunteers and agents from any and all claims, liability, fines or other matters arising from such attendance or participation: the Participant herby agrees to indemnify, defend and hold harmless the ITA, its officers, members, volunteers and agent from any and all claims, demands, expenses and liability, whether for personal injury, death, property damage, violations of law, or otherwise which is caused by the Participant and which in any way arises of or relates to the ITA function or activity. Note to Non-English Speakers: This is a legal waiver of liability. By agreeing to the above, you herby release ITA from any and all risks from participation in this activity. Your agreement acknowledges and confirms your full understanding of such waiver.
